If you have not received prior funding from The Couch Family Foundation, a letter of intent must be submitted prior to submitting a full proposal for funding.
Please complete the information below and submit it to the Foundation either by post or email.
The Letter of Intent should include the following:
- Letter submission date
- Organization name, address, phone, fax and website
- Organization's tax exempt number
- Contact person's title and e-mail address
- Brief description of your organization's work and mission (3-5 paragraphs)
- Brief description of what the funds being sought will be used for (3-5 paragraphs)
- Total program/project budget
- Funding amount requested
- Total organizational budget
- Number of years your organization been in operation
- Percentage of donations that go directly to your mission versus overhead/administration
- List other funding support
